Key ResponsibilitiesMedical Coding & Documentation Review• Assign accurate ICD-9-CM, CPT-4, and HCPCS codes for Inpatient, Outpatient, and ED encounters.• Review clinical documentation, physician notes, operative reports, and diagnostic findings for correct code assignment. • Validate coding for medical necessity per payor and federal guidelines. • Ensure coding accuracy to support appropriate reimbursement and reduce denials. • Query providers regarding incomplete or unclear documentation. Compliance & Quality Assurance• Adhere to CMS, AHIMA, AHA, and facility-specific coding standards. • Maintain compliance with federal, state, and payor billing regulations.• Stay current with coding updates and policy changes. • Participate in internal audits and quality improvement activities. • Meet established productivity and accuracy benchmarks. Required Skills• Advanced knowledge of ICD-9-CM, CPT-4, HCPCS, and multi-specialty medical terminology. • Strong understanding of anatomy, physiology, disease processes, and clinical workflows. • Proven experience in acute care coding, including:• Inpatient• Outpatient• Emergency Department (ED)• Ability to interpret and apply complex coding guidelines.• Strong analytical, critical-thinking, and problem-solving skills. • Excellent accuracy and attention to detail.
