Key Responsibilities:• Expert Triage: Conduct comprehensive telephone and video-based patient assessments using clinical protocols and critical thinking skills• Care Transitions: Manage post-discharge follow-up, medication reconciliation, and care coordination to prevent readmissions• Clinical Communication: Respond to EMR alerts, route messages appropriately, and facilitate communication between patients and providers• Documentation Excellence: Maintain detailed, accurate documentation of all patient interactions and clinical decisions• Team Collaboration: Work seamlessly with on-site clinical teams, physicians, and virtual care colleagues• Quality Assurance: Participate in quality improvement initiatives and maintain compliance with telehealth standardsRequirements:Required:• Active RN license in Texas or Compact State licensure• Minimum 3-5 years of clinical experience in acute care, emergency medicine, or telehealth• Advanced proficiency with EMR systems and remote communication technologies• Exceptional critical thinking, assessment, and time management skills• Reliable high-speed internet and dedicated home office spacePreferred:• Telehealth or telephone triage experience• Additional certifications in specialty areas• Bilingual capabilities (English/Spanish)• Experience with care coordination or case management Apply tot his job
