Telephonic Care Management Nurse (LVN, LPN, RN)

Skills• Active and unrestricted NYS LVN, LPN, or RN license. • Strong knowledge of chronic disease management, including chronic kidney disease, diabetes, COPD, and heart failure. • Ability to problem-solve and recommend solutions for care barriers.• Experience working with Electronic Health Records (EHR). • Ability to type 60 words per minute. • Bilingual in Spanish is a plus! Responsibilities• Work collaboratively with patients, families, and providers to develop personalized care plans and goals. • Provide outreach and engagement to ensure patients stay on track with their treatment plans. • Offer patient education to support self-management of chronic conditions. • Identify barriers to care and assist in addressing them. • Coordinate appointments, referrals, and community resources (e.g., home health, durable medical equipment).• Encourage and guide patients toward healthy lifestyle changes. • Maintain an assigned caseload of patients, monitoring their health and progress. • Conduct outbound calls to assess health status and provide guidance under provider direction. • Document patient interactions accurately in the Electronic Health Record (EHR). • Keep the care team updated on new patient developments. • Participate in ongoing training and professional development. • Perform on-call duties as assigned. Qualifications• At least 3 years of experience in Care Management, Case Coordination, home care, inpatient settings, physician practices, or in-home case management.Education Requirements• Relevant nursing degree (LVN, LPN, or RN). Experience Requirements• At least 3 years of experience in relevant nursing roles, particularly focusing on care management and chronic disease management.
