ResponsibilitiesThe RN-Patient Navigator in Ambulatory Care is a core member of our clinical transformation team, serving as a critical liaison for patients and their PCPs and/or specialists within our evolving ecosystem of connected care programs. This role provides end-to-end operational and clinical support for centralized virtual care models providing telemedicine in multiple states, ensuring a seamless, high-quality patient journey from pre-visit preparation through post-visit follow-up.The navigator will also contribute to the development and support of future care models, including virtual-based population health initiatives, and will be responsible for orienting and caring for patients new to Ascension. Demonstrated experience in clinical triaging, preferably in an ambulatory or virtual care setting, is highly desirable. Patient Navigation & Support• Utilize strong clinical judgment and triaging skills to assess patient needs, prioritize urgent issues, and escalate to appropriate providers or resources when necessary.• Proactively engage with new and existing patients to confirm their readiness for virtual visits across various connected care programs, verifying clinical information and providing guidance on technology setup. • Serve as a central point of contact for patients and their PCPs and specialists to manage post-visit needs. This includes managing a centralized clinical inbox to address patient questions, resolving medication and prescription issues, and coordinating the completion of lab, imaging, and referral orders.Clinical & Provider Enablement• Provide direct support to clinicians and care teams in new care models, helping them navigate workflows and addressing operational questions to ensure they can practice efficiently and at the top of their license. • Contribute to the development and support of new virtual-based programs as they are launched, demonstrating adaptability and a commitment to continuous innovation. Process & Program Management• Actively track and monitor operational data and patient outcomes for new ambulatory initiatives.• Make recommendations for process improvements and support the day-to-day operations to ensure key performance indicators (KPIs) are met. Complexity of Work• Within the scope of this role, the RN-Patient Navigator must exercise exceptional critical thinking skills and decisive judgment while working with minimal supervision. The work requires a high degree of proactivity, problem-solving, and adaptability in a fast-paced, virtual environment. The navigator will be responsible for managing patient needs across a wide range of clinical scenarios and will need to collaborate effectively with both centralized care teams and local, market-based clinical practices.RequirementsLicensure / Certification / Registration:• Registered Nurse obtained prior to hire date or job transfer date required. Licensure required relevant to state in which work is performed. • BLS Provider preferred. American Heart Association or American Red Cross accepted. Education:• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
