Title: Hospice Phone Triage RNPay: $42.34 to $50.81 per hourSchedule: PRN, minimum 2 shifts per month.Typically 12 hours each. Location: Mostly remote, must reside within Colorado, Florida, or Georgia. Will require occasional training near, and there will be hiring preference for, the Lafayette, CO area. JOB SUMMARYTRU Community Care is looking for a phone triage nurse. This nurse will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary. ESSENTIAL DUTIES• The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks.• Provides nursing care to meet the patient’s needs in accordance with the patient’s plan of care. • Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on-going plan of care appropriately to the interdisciplinary team (IDT).
