Job Description:
• Receive and process referrals for hospice care from the Guiding Light Hospice marketing team members, healthcare providers, families, and other sources.
• Verifies insurance eligibility and coverage including obtaining initial authorizations for services and seeks reauthorization for ongoing or additional services.
• Coordinate with healthcare professionals, including physicians, nurses, and social workers, to gather necessary demographic and medical information and other documentation.
• Communicate effectively with patients and their families to explain the hospice program, services offered, and expectations for care.
• Works with members of the billing team to ensure that managed care customers are billed according to managed care organization agreements.
• Establishes and maintains positive working relationships with current and potential managed care customers.
• Promptly responds to managed care customer’s requests and concerns.
• Assists in building community, customer, payer and patient perceptions of Guiding Light Hospice as a high-quality provider of services.
• Coordinate scheduling of admissions and ensure timely transitions to hospice care.
• When applicable, coordinate scheduling of hospice nurse practitioners for completion of initial face to face visits.
• Collaborate with insurance companies to verify coverage and obtain authorizations for hospice services.
• Maintain accurate and confidential patient records, ensuring compliance with regulatory standards and organizational policies.
• Provide ongoing support and assistance to patients and families throughout the intake process, addressing questions, concerns, and emotional needs.
Requirements:
• Bachelor's degree in social work, nursing, healthcare administration, or related field (preferred)
• Previous experience in hospice, healthcare coordination, or patient advocacy
• Strong interpersonal skills and ability to communicate effectively with individuals from diverse backgrounds
• Compassionate and empathetic approach to supporting patients and families during difficult times
• Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities simultaneously
• Knowledge of medical terminology, healthcare regulations, and insurance procedures
• Proficiency in computer applications, including electronic medical records systems and Microsoft Office Suite
• Ability to maintain confidentiality and adhere to ethical standards in handling sensitive information
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
