Job SummaryWe are seeking an experienced Licensed Social Worker to in a remote medical case management and advocacy role. As a key member of our team, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs.Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication methods to empower members to make positive lifestyle and medical choices. Responsibilities* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs* Research available options and resources for members with financial, transportation, safety, and housing needs* Empower members to make positive lifestyle and medical choices through education and support* Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health* Utilize telephonic and electronic communication methods to communicate with members* Maintain accurate and up-to-date records of member interactions and case management activities* Participate in ongoing education and training to stay current with best practices in medical case management and advocacyRequirements* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence* 2+ years of experience in medical social work or case management* Master's degree in Social Work or related field* Strong communication and interpersonal skills* Ability to work independently and as part of a team* Strong problem-solving and analytical skills* Ability to maintain confidentiality and handle sensitive information* Proficiency in electronic communication methods, including email and phonePreferred Qualifications* Experience working with diverse populations, including children, adults, and older adults* Knowledge of community resources and services for medical, financial, and behavioral health* Experience with case management software and electronic health records* Certification in medical case management or a related fieldSkills and Competencies* Strong communication and interpersonal skills* Ability to work independently and as part of a team* Strong problem-solving and analytical skills* Ability to maintain confidentiality and handle sensitive information* Proficiency in electronic communication methods, including email and phone* Strong organizational and time management skills* Ability to prioritize tasks and manage multiple cases simultaneously* Strong attention to detail and accuracyCareer Growth Opportunities and Learning BenefitsAt blithequark, we believe in investing in our employees' growth and development.We offer a range of training and education programs, including:* Ongoing education and training to stay current with best practices in medical case management and advocacy* Opportunities for professional development and advancement* Mentorship and coaching from experienced colleagues* Access to industry-leading resources and toolsWork Environment and Company CultureWe are a remote-friendly organization, and this role will be performed from a home office setting.
