As a Case Management Specialist, you will play a critical role in coordinating patient care and health services, working closely with our members, providers, and internal stakeholders to deliver high-quality care and support. Your mission will be to use your decisive thinking and judgment to facilitate the case management process, ensuring that our members receive the best possible care and support to achieve their health and wellness goals.Key Responsibilities Conduct comprehensive assessments of our members' needs and eligibility, using care management tools and data analysis to inform your recommendations and develop personalized care plans. Identify high-risk factors and service needs that may impact member outcomes and care planning, referring members to clinical case management or crisis intervention as needed. Coordinate and implement allocated care plan activities, monitoring progress and adjusting plans as necessary to ensure optimal results.Collaborate with case administrators, supervisors, Clinical Directors, and other healthcare professionals to overcome barriers to meeting objectives and objectives, presenting cases at case conferences to obtain multidisciplinary review and achieve optimal outcomes. Identify and escalate quality of care issues through established channels, using your influencing and persuasive communication skills to secure appropriate decisions and services that meet our members' needs and healthcare requirements.Provide coaching, information, and support to empower our members to make informed decisions about their healthcare and lifestyle choices, promoting healthy behaviors and self-care practices that support their overall well-being. Facilitate member engagement and participation in healthcare navigation, ensuring that our members are actively involved in their care and have the knowledge and skills they need to make informed decisions about their health and wellness. Use case management and quality management processes in compliance with regulatory and licensure requirements, company policies, and procedures, maintaining accurate and timely documentation of all care activities and outcomes.Essential Qualifications To be successful in this role, you will need: 3+ years of experience in a health-related field, with a strong understanding of care management principles and practices. A Bachelor's Degree or equivalent experience, with a strong foundation in healthcare, social work, or a related field. Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders. Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions in complex and dynamic environments.Proficiency in care management tools and technologies, with the ability to learn and adapt to new systems and processes. Preferred Qualifications While not required, the following qualifications are highly preferred: CRC, CDMS, CRRN, COHN, or CCM certification, with a strong commitment to ongoing learning and professional development. Experience working with Medicare and Medicaid populations, with a deep understanding of the complex needs and challenges faced by these individuals. Managed care experience, with a strong understanding of care management principles and practices in managed care settings.Experience working with geriatric special needs, social health, and disability populations, with a strong commitment to delivering culturally competent and person-centered care. Knowledge of assessment, screenings, and care planning, with the ability to apply this knowledge in complex and dynamic environments. Bilingual skills (English/Spanish; English/Creole), with the ability to communicate effectively with diverse populations and stakeholders.
